[Terraria: Eye of Cthulhu- personal redesign]
Instead of being literal eye that turns into a mouth, here it's a "balloon-like/ fish-like" animal flying via flesh bags filled with light gases, hidden inside their bodies. They use their lips to mimic apearance of an eye in attempt to scare off other predators similarly to how in real life various animals (most commonly insects) use patterns on their bodies for the same sake.
