Hmm, I want to try to weigh this accordingly. Seeing as how this is a sketch, rather than a fully realized product, I want to do my best to appreciate this for what it is.
Both the vision and originality for this piece will remain unaltered by that fact. Both Cassia and Clove seem so... organic here (no pun intended). Despite Clove's steeled demeanor, she's struggling a bit to put on a strong face for her sister. The tiny lip quiver and small bead of sweat speak volumes as she leans in for a private moment. All the while, its heavy contrast with the ever menacing Eggman makes the whole thing a study in oppression and hope, all wrapped in a bleak prognosis. Pretty heavy stuff for just a sketch.
I like how this one planned out too. The angle for the image, with the touch of isometric perspective and a small dip in the horizon line makes for a piece where the emotions can carry the day. The usage of the hash lines and hospital robes sell the backstory without having to explain a thing. I also really like how you used a few well placed details to show Cassia's debilitation's. Her eyes are clearly off in comparison to her sister, which is perfect. Good usage of lines for this sketch, its a testament to your technique. You do such a good job emoting with ears, and I would have liked to see Clove's ears show a bit more of the tensions in her heart, to match the fantastic impact that Cassia's ears show here.
This picture also has pretty good impact. For a sketch, it does hit you pretty hard in the feels department. There is the sisterly bond on display, there is the idea of hope and false hope and there is an absolute bearing to the image that makes it something special. I did want to dock it a bit here, as its unfinished, but I hope you get this back to finishing this one at this point.
I do find it odd that you neglected to include your horizon line and pinch point for this sketch. I would have loved to see you add the railing that Clove is leaning on, if only as a basic definition of perspective. Perhaps that's just not how you work, but a simple straight line like that does wonders to draw the eyeline and keep your perspective consistent throughout the image.
I like that you went out on a limb to ask for some critical feedback on a sketch. I hope some of what I had to say helps and keep up the good work.

I quite like the looming effect of Eggman's mug here. Speaks a lot without needing to explain anything.
