This is the form needed to purchase a Portrait Animation from me. Once you have a place on the list please download or take a screenshot of this form to fill it out in paint or some other program and send me a link to the finished form. The price of each option is the small number in the box. Not all of the options have a cost.

I shrink them by exporting the animation as a series of PNG images then load them into Photoshop, crop it and save it as a GIF.
