I don't remember Caramel and Big Mac being related, other than a very, very brief cutie mark switch. He's never canonically mentioned as family, and he's not one of the ponies at the Apple Family Gathering. Now, if it was Braeburn and Big Mac, I could see getting up in arms about relation.
