They're called plugs - to wear plugs that big, you have to gradually increase the size, or 'gauge' of the jewelry in your ear piercing, a little bit at a time. Over time, the ear lobe stretches out around the jewelry.

I love how the background is blurry and a little chaotic but sorta blends in nicely with the texture of the focus, like the smoothness of the hair and it's light wavyness~ I just wanna grab a fistfull of it!X3 (not to be creepy in ANY way...) o . o
The tones are to die for as well; this would be cool to draw with pencils~
