So I enter like a boss, not even looking while it loads and when I turn around... !!! Outrage! If this is not Sparta it can only be one thing.These screenshots should explain, last one contains a detailed description:

But really, this has happened to me so many times... once in Blackreach! (twas a terrifying experience, to see falmer fall out of nothing).
