I think he is not happy from Yahiko's death.Although he suffers so much,Konan still wants him alive very much.
About heath,maybe Nagato is not so painful or so skinny before Konoha battle.He use too much energy to fight so he nosebloods and looks very tired to fight Konaha.
Anyway,when Konan sees him died and find satisfying answer,she looks happy and comforted too.
