this is a piece about depression/insecurity, etc.I feel strong about this topic, having had a great deal of depression myself.
I always like to imagine a creature/monster that weighs you down on your shoulders, whispering negative things in your ears and making you slouch as you walk and sit and hurt your shoulders from the weight of itself(hence the red color in that place). it would be there, making you look down on the ground instead of people's eyes and slowly consume you, and make you more unsocial. make you scared, and insecure.

Certainly an avid display of one's demons, perhaps a useful way to help people understand what they can't see. Not entirely sure about some of the proportioning but as I can't draw to save my life, I'm in no place to comment.

I know my anatomy isn't a 100% and I do see mistakes.
but this is all from my head, and at the same time i don't strive to be 100% realistic with it.
