Really nice, detailed work, but I question the title you used for this. Was it meant to be a sort of sarcastic name? Or was it the title of the game?
What technique did you use to create this?

Well, it's not really sarcastic. The idea was that the main character would always have a psychotic looking smile on his face. Smiley was the only name I ever gave him. The game never got made but I will be making flash games in the future.
It was done in photoshop elements 3 using a graphics tablet.
