hm i mean for real not the middle tones,but the basic shapes in the photo

simply : the picture itslef blured in the mask a bit more contrasted, and each part of the pic separated by this selection are the hight tones and low tones approximated by the blur so it is smooth

eheh well observed, actually, the manip is :i created a gap in the middle tones (lighter the shadow and darker the light around the middle tones) and each side of the gap is merged to the other throw blured (but there is still the original picture under all)
