Uhm...Nobody using some kind of a flash game or something, if you mean it. We are do it by ourselves. Some people do it in SAI with vector layer, but me, I do it with a Pen tool.

How?! xD In Paint Tool SAI there is a vector layer with special tools. You can literally build an image point by point. You can do it by yourself, the is nothing difficult.
