It's actually really easy! Take a hard edged round brush at a small setting (like 6 px for the actual full res here) with no opacity settings on it, set it to white, work with your light source and draw a slightly trickling line down the side of the face whilst working with the cheekbone (or any other bones that may get in the way like a jaw or noseline). Add a lil shadow (I didn't do it here) against the edge of the tear depending on the lightsource.
