Can we talk about how well it's foreshadowed, though? It's one of those cases of taking something the player takes for granted and has learned to accept blindly and turning it on its head. Lots of games have companions that just kind of hang around you to provide tips, comments and plot exposition without it being obvious how they get there, maybe it's handwaved once or twice. "I'll find my own way around", yeah, sure, Ora, it's not like we had to swing through a near-impassable death trap to even get ONE of us in. And you just kinda instinctively don't question that. Even so, by the time the concept of outright hallucinations is introduced, you start to get that niggly feeling... meeting Dosan again seals it. The fact that he doesn't address Ora or even acknowledge her presence just makes it feel like there is something off without making it immediately obvious, and you start remembering all those times she found "her own way" into an impenetrable fortress or talked into your ear without being anywhere near you, and then you start to wonder if it wasn't more meaningful than just a gameplay conveience, after all...
Bonus points when you replay for the first time and realise that we never actually get to control the ninja before he gets his mark, and when he wakes up (presumably after getting the first mark, since he's having bad dreams and doesn't seem to be resting well), Ora is already there.
Or that time she convinced you setting fire to a large building with lots of people inside was a fantastic distraction... Oops.

I agree its kinda brilliant the foreshadowing they can just sneak by the player without noticing because its a trope and you don't think about why its happening that way. Very good use of an unreliable narrator. It was a shock to me the first time a hallucination guard left a body behind. I'm like wait they're hallucination how do they *realizes its a clan member's body* Gasp, what have I done.
