Even though I don't have a premium account, I'm just going to give a critique anyways. I really like how well you thought this out, about how so many people have a fear of dentists and what not. His eyes really stand out to me, make him look like a maniac, a psycho killer. His hair style looks good and all, but maybe next time use static electricity rather than hair gel? It makes it look like he's not that psycho. The hand with all the tools embedded was a good idea, but if you take a closer look then you will see that the tools are just shoved into his glove, I think if you could make it look more like it was actually growing out of his hand, it would look better. Also, I think that the drill should have been a colour other than red, and it should have blood on it, just to add to the effect. As for the coat, I think it should be slightly ripped to show struggle by the "victims". Also, no offence, but the blood spatters don't look natural. It almost looks as if it was carefully painted on. Try splattering it (if you didn't do that already), I also think the blood is a... bit over done, I don't think there would be that much, even if he was drilling through his head. Although, I could understand if he slit throats and... Off topic... But anyways, I don't think it should have been so heavily bloodied on his torso, some yes, not as much as his arms and his chest and face though. Speaking of which, I think his face should be slightly bloodied, and his face cover thing should be a bit off kilter, again to show struggle. I dunno if that makes sense but... That's just my opinion. Kthxbai.

considering as dentists get up close and personal to your face, blood on the torso is appropriate, and less blood on the arms would be better because they'd be holding their arms maybe up and away to get a better angle.. ya know... jus' sayin.

Makes sense. But still, it's not like it would be completely drenched in blood, think about it for a bit. I know whenever I go to the dentist, he's never leaning right over me, therefore his torso wouldn't be in the "splash zone". So I was just going by personal experience.
