I know this is a fetish fantasy, but you asked the question,so buckle up...
Being suspended in water for more than a day leads to skin damage. After ten days, you start getting blisters and skin cracking because of osmosis. That would lead to nasty infections because our bodies are petri dishes of bacteria. That's not even considering that this guy's skin is under strain from the rapid weight gain. And, he's being tube-fed a diet of 65,000 calories, primarily of sugar, daily. Beyond the severe risk of diabetes, if he's not getting any vitamins or protein, so he'll most likely develop scurvy. That would drop his red blood cell count and make it very hard for his body to fight infections. That, and his teeth would rot very quickly. And the all-sugar diet would spike and drop his blood sugar over and over until he fell into a diabetic coma.
Also, I just noticed that his nose is exposed while suspended underwater???
