A simple chart that RPDF Instructor Sergants have to fill after the evaluation of each RPDF Soldier....try to fill it ! it's nice and easy !

Are sabotage and espionage basic skills? If so, why isn't first aid in there? Or leadership, or comms, mechanics, and a whole plethora of other skills needed from a soldier to do various tasks.
How'bout this sort of idea: The sections are graded in points and these points would influence where a soldier would go for further training (logistics, espionage, specops, sniper, infantry, engineers, etc.).
There'd be physical ability (like you have there), combat skills (by range), other skills (demo, espionage, comms, int., medicine/first aid, tactics, etc.) and weapon skills (as shown in the chart).

Comms should include electronics (electronic door locks, hacking and the like), and might want to add mounted weapons to the arms list (HMGs, GMGs, heavy missile launchers [read TOWs], recoiless guns).
I could imagine there'd be another paper with an even more specialized list of skillz and weapons. Sort of as an addendum.

I believe that this version is mainly for his characters of the RPDF. Also hand-to-hand combat is a major part of CQB, is it not?

1- no this template is ALSO for RPDF anyone can do it !
2- CQB means short range fire fight even if hand-to-hand is really similar they are much different...

Iβll try to get to it soon, but the β profile portraitβ will give me some trouble. I have to do some thinking about what kind of gear is best suited for βmeβ and try to see if I can draw it. Maybe I will include asking you for a request to compose the character portrait for me, after I get the concept down.
