Duplicate the layer with the model, hide the original layer the erase everything except the skin you want smoothed. Now select filter-blur-gaussian-blur and blur at about 7px. Now unhide the original layer and adjust the opacity of the blurred layer to what looks best, usually in the 30% top 50% range but sometimes a bit higher. Erase any parts on the blurred layer that is blurring detail and finally merge down the blurred layer.
