My suggestions would be to make it smoother, with less cracks, squish the filling more so it's more filled looking, add "feet", and make the top more dome shaped instead of flat. I bake macarons a lot so I'm pretty familiar with them ; . I really like the way the cream looks, though :3

I think it looks very real as it is right now. (Especially the cream inside it) Maybe a more rounded top could make it seem more realistic. Like a hamburger bun top. I still think it looks absolutely delicious. ^_^
