Is that a space shuttle with an in-line payload + fairing?

Yeah, that's the low density/large diameter cargo container design. With that, you have to go up with a functionally empty cargo bay.
Edit: It gives you a maximum diameter of 10.6 meters. For larger diameter payloads, you have to replace the shuttle with a lenticular vehicle.
