Long story short on how she died. Zero killed her. In my opinion not on purpose. The reason they fought was because her brother the Colonel fought Zero and was killed.
The whole purpose of this war was due to Sigma who caused Sky Lagoon into falling into a city killing lots and lots of people and Repliforce was blamed for it. So the General leader of Repliforce decided to make a utpoia for reploids. However all the people of Repliforce were branded as Mavericks so X and Zero had to stop them. Thus leading into Zero fighting Colonel and killing him and which lead into fighting Iris which sadly got her killed.
That's the gist of what I got from reading a walkthrough, and watching the anime clips. As for the game.....I stink at it, I try though.
