edit: I think the creepy smile is Gaster or Chara, but Chara doesn't make as much sense as it does in the real game where you would probably have Chara following you (Frisk).

At least in game, the friend with the creepy smile is thought to be referring to Flowey. He's been following you the whole game up until that point, and disappears every time he's seen.

Tem Village. Best place in the whole game. Trying to hatch a hard boiled egg, Statue of Tem, Rich History of Tem, Tem Shop, Tem allergies (temmie pocks?), Tem spying on you, cynical dancing mushroom, and someone who actually lets you sell stuff to them. In Undertale, I'm always picking up stuff I don't need or find useless. Nobody wants the crap I find lying around, so when I finally find a village of adorable temmies who will buy just about anything, I get to sell every single item I've been dragging around in boxes or my inventory, and get rid of it all.
As to if it's Flowey or not, I don't believe so. He'd be too short to stand behind them and yet be seen behind them. They're too tall, and if Flowey is behind them in his standard flower form, you wouldn't be able to see him even if he were grinning like a creep. Therefore, I believe it's Gaster. Papyrus reacts wishing to know what the person means, and reaches out in a similar fashion to when he had a nightmare trying to save someone (Gaster). I think this parallel, combined with the fact Gaster could easily be seen standing behind them to those capable of seeing him, makes it pretty obvious it's Gaster. Of course, it's not nice to talk about someone who's listening so intently...
It's no wonder Sans is nervous. He couldn't be more anxious with Gaster around, even if now it only gives him a bad feeling. Papyrus, however, always tried to see the good in Gaster, and likely doesn't have such worries, seeking to understand and comprehend. However, Sans is more attuned to danger, and is right to want to leave for a safer location... Papyrus can't sense the potentially threatening presence, as he only sees the good in people...

I have two theorys on the creepy smile.
Its either flowey or gaster is my guess. As when Gaster falls into the void he turns into what we see as goopdad or just goopy gaster. And in so he has this gaster smile, but flowey also has in a way a similar creepy smile.
