Leviathan Form is Akuryo 's form accessed from the Leviathan Fuestle and granting him access to the Sinister Envy Bow Leviathan Cross. In this form, the Omnilens and Akuryo's chest/left arm armor turn into shade of turquoise called Leviathan Aquamarine which places Akuryo under the influence of Mido.

Akuryo's finisher is called Netami Hydro Wave. When it is performed, the weather turns cloudy while the hazy moon has formed. The background is surrounded by the ocean tides, causing the enemy to be trapped underwater. While the enemy is drowning, Akuryo locks onto his target, performing the Rider Shooting by sniping on its head.

Etymology
- Mido's name comes from the Japanese word for "green" (緑 mido), which it derived from the idiom, "green with envy".
- Netami is the Japanese word for "jealousy" (妬み netami).
