Yes, it was primarily done in Mypaint with some work done in GIMP.

It's sort of both. When I first started doing digital painting, I had a laptop that just didn't run Photoshop or Adobe very well- it simply didn't have the processing power. I found that GIMP and Mypaint worked just fine on it, though. Now that I have a much more powerful computer, I have gotten used to Mypaint and GIMP and actually prefer a lot of the features they offer. I also love the idea of open-source software and the communities that form around them, improving them and making them more powerful and flexible. I will probably start doing more in PS if only because the industry expects a certain familiarity with it, but the brush dynamics leave something to be desired IMO.

Creepy but wonderful detail. it looks like a genuine creature. I always wondered what a creature without a forehead looked like. Nailed it!

I hope you are considering making this project into a leather-bound (flesh like) book for sale? I will buy a few copies for sure. Something like the necronomicon from Army of Darkness.

If there are any publishers interested in doing something like that, sure. At the very least I will publish this as a book in some form of hard copy.

Pretty much anything that is a creature type, of which there are multiple members, rather than an individual (like Cthulhu or Nyarlethotep or Brown Jenkins for instance). I will probably make a compendium of Lovecraft mythos deities at some point as well, though.
