just suggestion... maybe it's better to make the hand shake with a real photo, then the red ribbon with paint or vector..

Great image
I really like the red ribbon against the hands in B&W.
If you want it to look more realistic try airbrushing dark gray or black with very very low opacity under the ribbon layer, making a barely visible shadow on both sides.

if you onle read the red words from the lowest one upward, it says... "KEEP AIDS NOT WORD"... that could be mistaken for some subliminal message!
